Chronic Conditions Management Demonstrators Evaluation Report.

Summary: Evaluation of the Health Guardians project which ran in Gwynedd from September 2009 to March 2011. The CCM Demonstrator project in Gwynedd aims to provide advanced learning and testing of new approaches to care for people with chronic conditions. The Health Guardians project’s main aim was to recruit volunteers to support people with long-term health conditions. Three elements of support were identified: listening, befriending, and signposting. The service users and volunteers valued the listening and befriending side of the service and the project revealed a need for befriending activities in this area of North Wales.
